Welcome to the Health Office

The health office is staffed by Karen Reid and Robin Woods-Brisson.

We hope everyone is having a healthy and relaxing summer. As fall approaches, we would like to send a few reminders regarding the paperwork necessary for us to maintain a safe and helpful environment for all students.
            Please find enclosed the health information and emergency contact form. This form must be completed and returned to the main office no later than Friday, September 18, 2009.
            Included on this form should be a list of any and all allergies, medical conditions, and prescription or daily medications, including dosage and times. You may attach a separate sheet with this information if necessary.
            In addition, if your student requires medications at school, there is a separate form that must be completed, with one section to be completed by the doctor. The doctor’s order must accompany this form and all medication to be given at school by the nurse must be in a pharmacy labeled container and must remain locked in the heath office per our district’s medication policy. If your student requires an in inhaler, a form can be obtained at the health office. This form must be completed in order for your student to carry the inhaler with them at school and school sponsored activities. These forms can be printed from the MTSD website under health office. No medications, including Tylenol and Advil, will be given to any student without these completed forms. NO EXCEPTIONS
            We also ask that any changes in your student’s medical condition, medications, or if there have been medical procedures, including surgeries and hospitalizations, that it be reported in writing as a brief note of explanation to our health office upon your student’s return to school. This information is vital if we are to provide your student with proper and safe care.
            If your student requires a 504 plan to meet their academic needs in relation to their medical condition, please contact your student’s guidance counselor to receive the necessary paperwork and set an August or September meeting date and time. If your student has required one in the past, please contact your previous facilitator. We ask that an updated, or current medical care plan and your doctor’s orders, including current medications, and any emergency information and phone numbers be included and be brought to this meeting. Providing this information at the meeting will help provide a continuum of care between home, school and your physicians, and enable the nurses to appropriately treat your student in both emergent and non-emergent situations at school.
